Neutral zone impression technique is one of the alternative approaches in managing mandibular complete
denture on a severely atrophic ridge. The neutral zone is a specific denture space where the force of
musculature such as lips and cheek are neutralised by the force generated by tongue therefore a zero net
force zone. This technique allows identification of the neutral zone and the mandibular denture constructed
is seating in a position in harmony with the surrounding musculature, ensuring the denture stability which
improves its retention. An 89-year-old female patient with severely atrophic mandibular ridge presented to
the student polyclinic with loose and unstable mandibular complete denture. After definitive impression
and maxillomandibular relation record, a base plate with retention studs was constructed and the neutral
zone was recorded using medium bodied silicone material at previously determined occlusal vertical
dimension. The patient was instructed to perform functional oral movements during the record. Based on
the neutral zone impression, the acrylic teeth were set within the stipulated zone and the polished surface
were waxed accordingly with the guide from a putty matrix. The patient was satisfied with the prosthesis
as it has improved retention, stability and comfort. In conclusion, this technique potentially improves the
patient’s satisfaction in selected cases and it is possible to be performed by undergraduate students under
close supervision.

Introduction: Different options can be considered to replace missing anterior teeth. In the management
of a traumatically or congenitally missing anterior tooth, resin-bonded, fiber-reinforced composite bridges
as a short- to medium-term temporary prosthesis offer numerous advantages over traditional bridges and
implant treatment. It is now possible to create solutions that are superior in terms of their aesthetic quality
as a result of scientific advances in adhesive technology as well as the utilization of tooth-colored materials
in their fabrication. In order to guarantee the long-term clinical viability of fiber-reinforced composite
bridges, one of the most essential things that must be done is a careful selection of cases to undergo this
treatment approach. This case report describes the management of a patient with a minimally invasive
direct fiber-reinforced composite bridge.
Case report: A 16-year-old male was referred from the orthodontic specialist clinic for a fixed replacement
of a missing anterior tooth. The patient had a history of trauma that resulted in the avulsion of tooth 21
and ankylosis of tooth 11. The treatment prescribed includes the provision of a chairside fiber-reinforced
composite bridge on teeth 11 and 22 to replace tooth 21.
Clinical significance: Direct fiber-reinforced composite bridges are a minimally invasive treatment approach
and can offer an alternative treatment option for patients in the short- to medium-term while they wait for
implant treatment later in life.
