Sinus Floor Elevation Procedures; A necessary need at times – A 15-years Follow-up
Placement of implants in the posterior part of the maxilla is frequently compromised or at times, impossible due to atrophy of the alveolar process, poor bone quality and
maxillary sinus pneumatization (Tadjoedin et al., 2003). Therefore, vertical alveolar ridge augmentation is often necessary before or in conjunction with placement of
implants.
For more than 30 years the maxillary sinus augmentation & grafting has been a mainstay of implant-directed maxillary reconstruction. Maxillary sinus augmentation is a
well-established procedure for functional rehabilitation of partially or completely edentulous patients (Boyne and James 1980). Alternatives such as short implants,
although shown to be effective in the short term, lack long-term studies to support routine use.
Various surgical approaches comprising elevation of the Schneiderian membrane have been proposed in order to achieve the necessary vertical height of the alveolar
process for the placement of implants with a sufficient length, including the lateral window technique, osteotome-mediated approach and sinus membrane elevation
without the use of a graft material. The survival rate for implants placed in grafted sinuses showed comparable results to those generally reported for implants placed in
pristine bone in the non-grafted posterior maxilla (Wallace and Froum 2003).
This presentation will review the fundamentals of maxillary sinus elevation including anatomy and physiology of the sinus, indications for surgery, preoperative evaluation,
surgical techniques, and management of complications. With preparation, education, and experience, the maxillary sinus augmentation/elevation is a procedure that
greatly benefits the patient, with a predictable outcome.
