Title of Presentation: Oral lichen planus an association with Sjogren syndrome

Background: Lichen planus is a chronic mucocutaneous inflammatory disease affecting mucous
membranes and/or skin of unknown etiology. Oral lichen planus (OLP) presents with a wide spectrum
of clinical and histopathological manifestations. Sjögren's syndrome is a systemic autoimmune disease
characterized by marked reduction of exocrine glandular secretions. Patients with Sjögren's syndrome
have 30% prevalence of OLP and oral lichenoid reaction. Treatment of OLP is aimed primarily at reducing
the length and severity of symptomatic outbreak.
Case Report: A 71-year-old female presented at Oral and Maxillofacial Department, Slim River Hospital
with complain of burning sensation over entire mouth for 3 months. She has underlying hypertension,
dyslipidemia and peptic ulcer. Upon intraoral examination noted patient to have dry oral mucosa with
erythematous area and white striae over bilateral buccal mucosa, erythematous area over upper and lower
labial mucosa and whitish plaque over left lateral border of tongue and right ventral surface of tongue.
Incisional biopsy was carried out and histopathological examination interpreted as oral lichen planus.
Treatment was carried out for this patient which included topical application of corticosteroids then systemic
administration of corticosteroids when there were no signs of improvement of lesions. Rheumatology clinic
follow up by patient, confirmed the patient has Bechet disease with Sjögren's syndrome. She is currently
under management for her OLP symptoms. Diagnosis of OLP can be achieved by clinical and histopathology
examination. However, more investigations are required for cases which do not respond well to treatment
to exclude other autoimmune diseases.
