Introduction: Following tooth extraction, progressive resorption of the alveolar ridge will take place. This
may sometimes lead to severe atrophy of the alveolar ridge and subsequent problems in placing dental
prosthesis. Alveolar ridge preservation (ARP) can be considered to preserve vertical and horizontal
dimension of the alveolar ridge following tooth extraction which will aid in future implant placement while
preserving aesthetic component in the aesthetic region.
Objective: To restore and maintain the function of splinted crown which has been converted into cantilever
bridge following extraction of one of the abutment teeth while maintaining the aesthetic appearance.
Methodology: A healthy, 49-year-old male patient was referred by a GDP for management of his
periodontal conditions. Intra-orally patient has localized periodontitis, further examination revealed that
he has a splinted crown on 22 and 23. 22 has a poor prognosis due to gross carious lesion at the crown-
root area. After management of periodontal disease, 22 was atraumatically extracted, followed by alveolar
ridge preservation using alloplastic bone grafting material (Ethoss). The tooth structure of 22 within the
prosthesis was removed and replaced with GIC. Case was reviewed after 1 week, 2 weeks, 1 month and 3
months after the surgery.
Result: At 3 months review, clinical findings showed a positive outcome with an improvement of the
surrounding mucosa and adequate preservation of vertical and horizontal contour of the alveolar ridge.
The cantilever bridge has adequate function and aesthetically pleasing.
Conclusion: In the aesthetic region, ARP following tooth extraction helps in maintaining both vertical and
horizontal dimension of the alveolar ridge and indirectly the aesthetic component.
