Introduction: The bilobed flap is a double-lobed transposition flap commonly used in reconstructive
surgery due to its versatility and effectiveness in achieving optimal cosmetic and functional outcomes.
This case series illustrates the application of bilobed flaps in the reconstruction of various facial lesions,
demonstrating its adaptability and success in oral and maxillofacial reconstruction surgery.
Case Description: This series includes cases from an adolescent and two elderly patients.
Case 1: A 68-year-old male with basal cell carcinoma on both the chin and nasolabial area. The nasolabial
lesion was excised and reconstructed with an advancement flap, while the chin lesion was excised and
reconstructed with a bilobed flap. Case 2: A 79-year-old male with squamous cell carcinoma in situ on the
right pre-auricular area underwent an excisional biopsy with a 4mm margin, followed by reconstruction
using a bilobed flap. Case 3: A 14-year-old male with a melanotic nevus on the chin. The nevus was excised,
and a bilobed flap was used for reconstruction. Ensuring a safe surgical margin for excision created a
significant defect, requiring reconstruction. The bilobed flap provided sufficient tissue for closure while
       ensuring satisfactory cosmetic results.
Conclusion: The bilobed flap is a versatile and reliable option for reconstructing facial defects following
the excision of various lesions, including squamous cell carcinoma, basal cell carcinoma, and melanotic
nevi. This case series highlights its utility in achieving excellent cosmetic and functional results in oral and
maxillofacial surgery. The bilobed flap's design allows for precise closure with minimal tension, making it
particularly advantageous in areas with limited tissue laxity. These cases highlighted the flap's adaptability
and effectiveness, reinforcing its value as a crucial technique in the reconstructive surgery.
